Cremation form for body organs
To cremate a body organ that has been retained after an autopsy of a remains, please find below two different methods of supplying the necessary paperwork required by the medical referee.

- Letter of authorisation from the executor or next of kin
- Letter of clearance from the pathology dept of the hospital where the autopsy took place stating the patient number and autopsy number
- The Form D from the original cremation if signed by a coroner.
