Where are they sent to?
Once the cremation forms are completed they are to forwarded onto the Mount Jerome office as soon as possible. There are three ways to do this:
- Email address: firstname.lastname@example.org (Need file size of <500KB!)
- Fax number: 01 496 0994
- By hand to Mount Jerome
Please note that the email method is by far the most efficient of the three. The Medical Referee has instant access to the cremations forms. This affords the Referee the opportunity to spot any difficulties with them before the remains arrives at Mount Jerome.
But to enable the Referee quick access, we must ask that the file size of all the scanned cremation forms be kept to less than 1 MB. Just scan them as a black and white document on your scanner.
