Cremation forms must be completed for each individual cremation of an adult, infant, still birth or a body organ. They must confirm the details of the deceased, the authorisation by the correct individual and the circumstances surrounding the cause(s) of death.
Please note that cremation forms are not death certificates. They are totally separate from the medical papers used to issue a death certificate.
